Eligibility for accommodations under Section 504 of the 1973 Rehabilitation Act, the Americans with Disabilities Act of 1990, and the ADA Amendments Act of 2008, are based on the existence of a physical or mental condition that substantially impacts academic and campus life. In order to receive reasonable accommodations, students must present documentation of a disability that meets the Office of Accessibility Services guidelines.
Documentation must be completed by a physician, a psychiatrist, licensed psychologist or a state or federal rehabilitation specialist. This documentation should be current at the time of enrollment or when academic accommodations are implemented, should describe functional limitations, and should include recommendations for academic accommodations appropriate to the disability. Changes in medical and psychological disabilities should be reported to the Assistant Dean of Students as they occur. Documentation for learning disabilities and attention deficit disorders must describe the students as an adult and can also be certified by an education specialist. If you have been diagnosed with a learning disability or an attention deficit disorder and received services when you were an elementary or high school students, please ask for a re-evaluation during your senior year. Documentation presented must be current for you as an adult. Documentation completed when you are a high school senior is acceptable.
Students should meet with the Assistant Dean of Students each semester to determine the academic accommodations necessary for the classes in which they are enrolled.
Persons with disabilities may be found eligible for accommodations after they have
- Become established as students.
- Presented documentation of the disability that meets Accessibility Services guidelines.
- Had the documentation reviewed by the Assistant Dean of Students.
- Completed an intake interview.
Note: Documentation accepted by Defiance College might not be accepted by other institutions, agencies, and/or programs (e.g. testing agencies, licensure exams, certification programs). Please check with the specific institutions and/or programs to determine their documentation requirements.
