To all Residential Students at Defiance College regarding Hepatitis B and Meningitis vaccines,

The 125th Ohio General Assembly and Governor Taft have passed House Bill 142. This institutes all students living in campus housing will be required to prove documentation that they have either received the Hepatitis B and Meningitis vaccines or have read the information on these diseases . You will not be permitted to live in campus housing until this documentation has been submitted. Print the Vaccines Received Form, fill it out and send to our school nurse.

We do provide both vaccines at the Defiance College Health Center for a cost to the students. If you have not received the vaccines and choose not to, please sign the Refusal Form acknowledging that you have read the information and decline to receive the vaccines at this time. We will keep this on file, and if you later choose to receive one or both vaccines, please notify the Defiance College Health Center.

It is not required that residential student's receive the vaccines, but is highly recommended. All colleges/universities in the State of Ohio who have students living in campus housing need to show documentation indicating the students choice declining the vaccines and that they are aware of the diseases and choose not to receive the vaccines.
