Incoming full-time students:
A Health Record (click here for the form) is a requirement for admission to Defiance College for full-time students. It is the policy of Defiance College to have in our medical files documented proof of immunization and a personal health history on each incoming student. This is for your protection as well as the other students and the community. It is important to the Health Center to provide you with the best medical care possible while you are a student at Defiance College.
The easiest method of obtaining your immunization record is from your high school, your medical records, or your County Health Department. If possible, obtain a copy and attach it to the form. If your health record and/or immunization record is not complete or up-to-date by registration, we will require that you receive the needed immunizations.
The Medical Form will be filed in the College's Health Center located on the second floor in the Serrick Campus Center.
Health insurance is mandatory. The student may stay on their parents' plan or take out the college insurance. The college medical insurance may also be purchased as a secondary insurance for the student during Orientation and Registration.
