____ Health Insurance Information (To show proof of health insurance, you must bring your insurance card with you for us to copy. If you are choosing the College's Health Insurance, the cost is $510 for the year.) Need a form for Personal Insurance Information?

____ Housing and Medical Forms (if you haven't turned these in previously)

____ Resident & Commuter Questionnaire

____ Outside Scholarships (from Awards Programs, etc.) (Outside scholarship checks, letters, or forms can be brought the day of Orientation but are preferred to be mailed of faxed (419-783-2579) to the Financial Aid Office prior to Orientation if possible.)

____ Information for loans: two personal references (address and phone number of grandparents, uncle, aunt, cousin, minister, etc.)

____ Official, FINAL High School transcript (After graduation listing final class rank and final grade point average)

____ Vehicle Registration Form (form mailed to you, fill it out to receive a parking permit)
