Visit Agreement

Contract Statement for Prospective Student:
As a visitor to Defiance College I fully understand the premise of my visit and all the activities scheduled. I have read my visiting agenda and am completely comfortable with its content. I also understand that I am not permitted to attend any social activities that are not organized and supervised by college personnel. If I participate in any activities outside of the visiting agenda, and or caught violating campus policies or state law, my admittance to the college will be jeopardized.

Contract Statement for Parent/Guardian:
I am the parent or legal guardian for the above participant and give permission for my child to attend. I understand and agree that my child will comply with the College’s rules, standards, and instructions. I understand and hereby acknowledge that I, on behalf of my child, assume all risks incurred from my child’s participation. I understand that I am responsible for my child’s medical or medication needs and further agree that in an emergency and/or if I cannot be reached, the College may take whatever action is deemed necessary with respect to my child’s health and safety. I authorize the College and its employees to place my child at their discretion and without my further consent, in the care of a medical professional for medical services and treatment. I understand that I will be responsible for any fees and expenses for any service and/or treatment.
